New Agents/ Suppliers must complete this form to get setup within the LCBO systems or to update information.
All new Agents must include a copy of the AGCO license for their first submission.
Form required to transfer direction of payment from LCBO - Supplier to LCBO - Agent.
All order submissions must contain a supplier quote. Only one supplier quote needs to be submitted per order (regardless of the amount of customers) as long as all products being ordered are declared in that quote.
Standard Frieght Rates will provide shipping costs per case from all ports all over the world. If you are shipping from a location that does not have a standard freight rate (or if you do not meet the minimum order quantity for Ex-Cellars shipments from a listed location), a freight quote template may be required from the supplier.
Lab Waivers for personal consumption exemption are available for products where the annual quantity ordered does not exceed 90L.
Lab Waivers for price exemption are available for products where the annual quantity ordered does not exceed 90L and the retail price of one unit exceeds $100.00.
An administration fee of $55.00 is applied to the agent for each Lab Waiver request (one Lab Waiver per product).
The supplier is responsible for all transportation related charges to Toronto, including arrival charges at destination airport Toronto YYZ such as Terminal, Handling and NAVCAN charges.
Delivery to Trillium Supply Chain ( 205 Speirs Giffen Ave., Caledon, ON L7C 3Y7) and any Canadian duties and customs clearance charges will be handled by the LCBO.
This document must be filled out and returned to customsdepartment@lcbo.com prior to the receipt of the goods at the airport. If the form is not returned or incomplete, there is a risk of encountering delays and incurring additional charges at the airport.
